Hi, Dr. Bobbi of Stanley Dentistry here. I wanted to talk to you today and give you some information about Invisalign. Invisalign are clear aligner braces that can help straighten your teeth without being intrusive in your personal life. I’m going to turn it over to our lead Invisalign assistant, Zara, to tell you more.
Invisalign is a clear aligner system that is a part of orthodontics that allows patients to be able to align their teeth for many different reasons. It’s very painless, almost virtually painless, and it is also pretty much invisible. You’re not able to see them as much as [traditional braces] and they’re usually faster. A lot of patients average maybe six months to a year of treatment. So, with regular orthodontics, it requires the placement of brackets and the use of metal wires to be able to move teeth precisely — which is a very effective way [to straighten teeth]. But, what’s great about Invisalign is that it’s not fixed in your mouth, thus you’re able to take it out and have very proper hygiene. It’s minimally invasive and it does well for a lot of our patients.
So, people who are candidates for Invisalign are generally people who have a wide range of malocclusion. Whether it’s minor relapses, maybe they’ve had orthodontics done in the past, or it could be used for some more major procedures such as if they’ve had teeth extracted and are interested in implants. Invisalign could also be good for patients who want veneers and they need to correct their deep bite. There are really very few limits to the kind of patient who would need or want Invisalign.
In the Invisalign process, there’s a set of aligners that we customize for your teeth specifically. You wear them for twenty to twenty-two hours out of the day which is the highest recommended effective time for them. You only take them out to eat and brush. Otherwise, you’re wearing them at all times to get the most ideal smile.
For your first Invisalign appointment, we’ll take an Itero scan. [The intraoral device] stitches together numerous images simultaneously to create a model. That way, there’s no need for gooey impressions. The process is very, very fast. We take [the scan] and take a series of photos and send them off to Invisalign to evaluate so we can get the best result for your smile. They correspond with Dr. Bobbi directly. This means they’re able to let us know of any adjustments, or if there’s anything you’re interested in changing as a patient, or if Dr. Bobbi wants to do something to perfect your smile. She goes back and forth with the lab and then they create your Invisalign aligners. We receive the aligners back from Invisalign and we bring you back into the office to make sure that they fit perfectly for you.
One thing that we might do at this first appointment is place attachments. They are little buttons of composite that go on the front side of your teeth. They serve specific anchor or rotation points for the aligners to make them more effective. Sometimes, doing this can cut down on the number of aligners you’ll need. At the very end of treatment, we polish those off.
At that first appointment, where you receive your first few sets of aligners, you go home and you continue to wear them and go about your life. Then your follow-up appointments will take place virtually. We provide a virtual platform and that way you’ll be able to check-in with us. We’ll be able to talk about how you feel like things are going and we’ll also be able to have you send pictures and check on your progress. We’ll be able to see how your teeth are tracking and if everything looks great, we’ll set up a curbside pickup for your next few sets of aligners.
This is great because it doesn’t require you to set up an appointment. You just swing by and we’ll make sure you get your next few set. Then we’ll continue to follow up with you virtually. At the end of all your aligner sets, we’ll talk about whether there’s more that you want to change — if there are minor adjustments that we want to make. If not, you’re all ready to get your retainers.
For those, you come into the office and we take one more scan of your mouth to get a perfect fit for your retainers. We send that off and the turnaround time is about a week. You come back in, we polish off all your attachments, and then we get you set up with your retainers.

At Stanley Dentistry, we pride ourselves on being one of the oldest family-owned dental offices in the Triangle. If you had braces earlier in life and now, once again, have some dental misalignment, is it possible to get Invisalign? Absolutely. This is actually a really common phenomenon in our office. After orthodontic treatment, you have to be diligent about either getting a permanent retainer or wearing your removable retainer every single night. Many of our patients lost or stopped wearing their retainers after their teenage years. Without that retainer keeping the teeth in the correct position, they gradually move back. Ten years without wearing a retainer can have surprisingly dramatic consequences. In severe cases, the misalignment is just as bad as it was pre-braces.
I cannot say for sure that you will be a good Invisalign candidate without an exam. Invisalign can fix many orthodontic problems but it still can’t do everything traditional braces can do. Tightly packed misalignment oftentimes requires braces. If you do need braces (after going through braces treatment once before), the length of treatment will most likely be shorter than your first stint with braces. Modern braces are smaller and faster than older ones. In most cases, adult patients are in and out of braces within a little over a year.

After the virtual consult, you’ll schedule an in-person appointment at our office in Cary, North Carolina. Invisalign appointments are very fast since all we need is a quick digital scan of your teeth. The scan takes a few minutes and is 100% pain-free. It’s sort of like getting an x-ray done — except even easier since no radiation is used to produce the image. We then send that scan off to the Invisalign lab. Within a week or two, they’ll send us your first batch of clear aligners.
Invisalign typically takes between six to twelve months to complete. Patients go through a series of clear aligner trays, changing them every week or every other week. We keep a close eye on every Invisalign patient’s progress to ensure a perfect final result. If you’re not happy with your smile at the end of treatment, Invisalign offers complimentary realignment trays to refine any minor flaws.
We want your smile to last a long time which is why we always strongly encourage our patients to get retainers. Our office offers both removable and fixed retainers. I’ll take a look at your case and recommend one or the other. I typically prefer to put fixed retainers on my patients since it ensures long-lasting alignment. Removable retainers work well — but it’s difficult to remember to take them in and out.
It’s weird, the way I came to see Dr. Stanley. See, I have a hypermobility syndrome that affects chronic pain throughout my body. I was searching for years and years and spent tens of thousands of dollars on different techniques to try and get this pain syndrome under control. Part of hypermobility, and controlling it, is having control over your structure because when you have hypermobility, your ligaments don’t hold you together very well. So, having structure, and strength in that structure, is very important. Dr. Stanley understands the relationship between the bite and the tongue and how it impacts the neck. Not only the physical structure but the neuromuscular structure functioning in that area.
I went to a neuromuscular dentist prior to Dr. Stanley and went into braces and it failed. I was actually in more pain than I was prior to, so I was very disappointed. I reached out to fifteen or twenty national experts across the country and no one really had an answer for me — but Dr. Stanley did. She said ‘I think I have something that will really help you.’
She expanded my upper palette and I have a frenectomy — a release of the tissue underneath your tongue — and then went back into braces the last time. And she helped me with the finances for that because she understood that this has been a long journey for me. It was one of those things that I can honestly say was a game-changer. There’s no magic bullet but having her as a part of my team is one of the best things to ever happen to me. I can’t talk more highly of Dr. Stanley — not jut as a dentist but as a clinician.
One of the biggest frustrations [going into previous treatment] was monetary. Everyone was selling me the magic bullet that was going to fix everything. It was a complex interrelationship of how my body was interacting with myself. She looked at me as a whole person — everything together. She seemed to have a mastery of the understanding of that. It put me at ease, immediately when I walked in.
This is something that I’ve been going through for ten years so my vocabulary and understanding of it is extremely in-depth. So, when I hear someone who can explain to me my symptoms and why this might be happening — when I finally have someone who can communicate with me in a way that I understand, and be compassionate towards me, and work with me…Not just in the sense that “in this alternate reality,” but to actually put something into practice, and plan for a successful outcome. To realize that, it’s just amazing. It’s wonderful.
When I say “game-changer,” it’s one of those things where, in my life, I was in a lot of pain and after that experience…getting my palette expanded, the braces, and everything moving into place — it really was a structural component. After I got out of braces, the tension was 70% less. When you have that relaxation of the tension and the muscles can contract and relax in a way that is neuromuscular-ly correct…I was able to incorporate exercises and do things that would reduce pain on down the kinetic chain. It was a game-changer from the top-down. I mean, I know a lot of times (I coach baseball), everything happens from the ground up! But, sometimes and in this case, it’s from the top down. She was really able to help me there.
So, primarily I worked with Brenda and she was just great, always in a wonderful mood, and they recognized my positive energy and they basically added to it. It’s a synergistic environment. That’s what I’m always looking for in a team — for people who can support who I am. So, that was, for me, the biggest thing. The office supported me personally.
TMJ is not always a strictly teeth thing. But, if it is related to your bite and it’s related to your upper cervical spine, then…in my personal experience and having seen seven or eight experts, I would [recommend] seeing Dr. Stanley first. I would not go to an upper-cervical specialist. I would rule this out first. If it’s a teeth problem that’s affecting your cervical spine and the relationships there that cascade down your body, I would go here before I went to any neuromuscular dentist in the state of North Carolina.”

Like our in-person consultations, our virtual consultations are 100% free. They offer patients some time with the doctors to ask questions about different procedures. The doctor may ask you to take some pictures of your smile so they can start putting together a customized treatment plan. When you think of orthodontic braces, you probably imagine a mouth full of unsightly (and sometimes painful) metal brackets and wires. After all, getting braces isn’t exactly an experience most of us would like to have. They create beautiful smiles but getting to that point isn’t always easy.
Many people still get traditional metal braces but, in 2020, there are a lot of other options out there too, depending on your orthodontic needs and your budget. Here are the four most popular kinds of braces that are currently on the market:
These are the braces we’ve all seen before. Traditional braces use metal brackets and wires to move teeth into alignment over an extended period of time. In recent years, some orthodontists have started using heat-activated arch-wires with traditional braces. These allow the natural heat in the mouth to help move teeth quicker than ever before.
Ceramic braces are very similar to traditional braces. They sit on the front of the teeth, just like traditional braces, but they blend in with the teeth a little better since they’re made of tooth-colored ceramic material. The wires can sometimes be colored too to help make the braces appear invisible.
They work just as fast and effectively as metal braces but they are typically more expensive.
Unlike traditional and ceramic braces, lingual braces attach to the back of the teeth instead of the front. This makes them virtually invisible but more uncomfortable than traditional braces since the tongue frequently comes into contact with them.
Lingual braces can’t fix everything so if you’re interested in getting them, you should first ask your orthodontist if they’d work in your unique case.
The most recent (and wildly popular) addition to the world of braces doesn’t look like braces at all. In the last fifteen years, Invisalign has cornered the orthodontic market by offering clear, pop-on retainers that straighten teeth without brackets, bands, or wires. Invisalign uses a series (18 to 50) of solely plastic trays to guide misaligned teeth into the proper position.
Patients who choose Invisalign have to wear the trays as often as they can to see the best results but, luckily for them, the trays are clear and fairly comfortable so it isn’t too much of a chore. You do have to take them out when eating or drinking dark liquids to prevent staining.
We’ve said it before and we’ll say it again: if you have dental insurance, the best way to save yourself a lot of money is to remember the “use it or lose it” principle. Every year, we run a campaign encouraging our patients to schedule their cleanings, fillings, and other dental services before the end of the year, since dental insurance doesn’t rollover. If patients don’t use the amount allocated by their insurance by the end of the year, they lose that money.
August probably seems a little early to begin thinking about the end of the year but schedules fill up quickly. Back-to-school is just around the corner and before you know it, the holidays will be here. Evade the unnecessary anxiety by scheduling early and using your dental insurance before it’s too late.
Dental insurance is an overly complicated web of unnecessary rules, insane numbers, and fine print. Very few of our patients truly understand how it works since the insurance companies do their best to keep anyone and everyone in the dark. They want their policyholders “out of the know” because that often means making more money. If you don’t know how something works, you won’t notice when the system cheats you.
In dental insurance, that cheat is losing the services you already paid for at the end of each calendar year. In a typical dental insurance plan, you probably get the following:
If you only get one cleaning in May and don’t go in for another one six months later, you lose the money you paid to the insurance company. The insurance company is happy because you paid them for a cleaning that they never had to cover. If you try to go in for a cleaning in January to use one of your two cleanings from the previous year, you’ll just end up using money from your current annual plan instead.
It is never a good idea to sit on needed dental work. Many serious operations, from knee replacement to open-heart surgery, require dental clearance so it’s best to stay abreast of any dental concerns. Of course, that isn’t always easy and we understand that. A lot of people push dentist visits to the back burner because of financial concerns. If you need a lot of treatment, it can get expensive.
The thing is, a lot of our patients come into the office for the first time not knowing that their insurance will cover the majority of their treatment. They assume that insurance won’t cover things like fillings, crowns, or oral cancer screenings when in reality, they will. If you haven’t already gone past your limit for the year, many dental insurance companies will cover a variety of services.
The best way to know what your dental insurance covers is to come into the office and meet with our insurance experts. They’ll give you a complimentary insurance check to see what insurance will and won’t cover.
If you have a dental insurance plan, you’ve already paid for regular visits to the dentist. You’ve already paid for the occasional filling or crown. Isn’t it better to schedule those appointments and effectively use that money instead of allowing the dental insurance companies to scam you out of hundreds of dollars?
We hear a lot of questions about dental health from our patients: “How do I get rid of bad breath?”,“When do I need my wisdom teeth removed?”,“Will my veneers stain?” Those, we anticipate — we’re a dental office after all. We love helping patients understand their oral health better. What might be a little surprising is how often we hear insurance-based inquiries. “Are you in-network with (fill in the blank here)?” is undoubtedly the most frequently asked question in our office.
Patients assume that dental insurance is comparable to normal health insurance because there’s similar verbiage. Unfortunately, the two share very few similarities. This leads to a lot of confusion, hence the inundation of dental insurance questions we receive daily. We’re here to clear the air on what exactly dental insurance is and how it can both help (and hurt) your wallet.
With health insurance, you’re paying the insurance company to cover the costs of potential injuries, check-ups, etc. Health insurance has its own problems but it typically does cover at least part of big, medical expenditures.
Dental insurance does not. If you’re paying for dental insurance, you’re essentially paying for a dental coupon. You have a certain amount of money (typically $1000) that you can spend on dental care for the year. If you break a tooth and need immediate care but you’ve already spent that $1000, your dental insurance will not cover the treatment. To make matters worse, if you go all year and only spend $200 on dental care, the remaining $800 does not rollover. You lose all that money.
If dental insurance is one of your employment benefits, use it. Schedule your six-month wellness visits and don’t let it go to waste. If you don’t have dental insurance, consider paying out-of-pocket. We know that sounds scary but in the long run, you will save money. Dental insurance companies stay afloat because they rely on people’s ignorance. They know that most people don’t know how dental insurance works.
